Community Healthcare Partnership
What is the CHP?
The Community Health Partnership, or CHP for short, is part of NHS Shetland and was established on 1st April 2005. The CHP is responsible for managing a wide range of community based health care services and, in partnership with lthe Shetland Islands Council (SIC), provides a focus for the integration between primary care, specialist services and with social care to ensure that local population health improvement is placed at the heart of service planning and delivery.
The CHP will:
- Support the improvement of the health of the local communities;
- Provide service benefits for local people;
- Work in partnership with the SIC, voluntary sector and other stakeholders;
- Actively involve the public, patients and carers in decisions that affect the planning and delivery of health care and health services for their communities.
The CHP will improve the way your community health and social care services are organised and delivered. These services will continue to be provided in local health centres, care centres, schools and community settings across Shetland. The CHP is also responsible for many services already delivered jointly by health and social work staff including: older people, learning disability and mental health services.
The CHP also works closely with local GPs, pharmacists, dentists, opticians and specialist staff from the hospital.
